Union Gospel Mission is seeking a Part-Time Case Manager in Fort Worth, TX to oversee day-to-day program planning, manage a residents caseload, and provide hands-on client-based support focused on housing, life skills, and recovery.

You will screen and assess residents, develop care plans, lead educational groups, coordinate referrals, and document progress. LMSW/LBSW preferred; minimum education and ability to work flexible hours are required.

Qualifications

• Five years of clinical Social Work/Case Management experience preferred.
• Bachelor's in Social Work or Ministry; experience case managing homeless persons can be considered.
• Texas LMSW preferred; minimum LBSW.

Responsabilités

• Screen and perform psychosocial assessment for incoming residents.
• Complete new resident intake process, including intake assessment.
• Develop care plans and individualized goals with residents; monitor progress.
• Provide residents with assistance regarding program progress and class attendance.
• Lead educational courses, developmental groups, and resident meetings.
• Coordinate referrals with outside agencies and resources.
• Maintain documentation for all phases and services provided.
• Interact with residents on life skills, daily living tasks, hygiene, etc.
• Develop and update a discharge plan and exit strategy.
• Participate in cross-functional support and communication for residents and peers.
