Job summary

Union Gospel Mission of Tarrant County in Fort Worth, TX is seeking a Case Manager to oversee day-to-day program planning and the implementation of a Residents caseload, providing hands-on client-based programming to support independent living, skill development, emotional healing, and spiritual needs.

We are looking for five years of clinical Social Work/Case Management experience, with a focus on serving single women and seniors; LMSW preferred, LBSW acceptable, and familiarity with MS Word,

Qualifications

• Five years of clinical Social Work/Case Management experience preferred.
• Experience serving the unsheltered community, particularly single women and seniors.
• Bachelor's in Social Work or Ministry; direct experience case managing homeless persons can be considered in lieu of education.
• Texas LMSW preferred; Minimum LBSW.
• Proficient in MS Word, Excel and Outlook.

Responsibilities

• Screen and perform psychosocial assessment for incoming residents.
• Complete new resident intake process, including intake assessment.
• Develop Care Plans and Individualized Goals with residents; monitor and document goal and care achievements.
• Provide residents with assistance and support as needed regarding program progress, class attendance, and behavioral issues.
• Lead educational courses, developmental groups, and resident community meetings.
• Connect residents to referral networks with outside agencies and resources.
• Maintain documentation for all phases and services provided.
• Interact with residents regarding life skills, daily living activities, and hygiene; provide coaching as needed.
• Develop, review and update a Discharge Plan and Exit strategy.
• Participate in cross-functional support and collaboration for residents and peers.
