Job summary

Crescent Hospice is seeking a Licensed Master Social Worker (LMSW) to provide compassionate social work services to hospice patients and families in Beaufort, Jasper, Hampton and Colleton counties. The role emphasizes counseling, psychosocial assessment, and collaboration with the interdisciplinary team.

Schedule is Mon-Fri 8am-4:30pm, with a focus on dignity, comfort, and support at end of life. Requirements include an MSW, state licensure as a Social Worker, and a valid driver\'s license.

Qualifications

• MSW from an accredited school of social work.
• Licensed as a Social Worker by the State Board.
• Valid drivers license, vehicle insurance, and access to a dependable vehicle.
• Minimum one year social work experience in healthcare; hospice experience preferred.

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with the Bereavement Coordinator.
• Attend interdisciplinary meetings and support care planning.
• Provide empathetic counseling to patients and families.
