Job summary

Crescent Hospice seeks a Licensed Master Social Worker to deliver compassionate social work services to hospice patients and families in Conway. The LMSW/MSW will provide counseling, assess psychosocial needs, and collaborate with the interdisciplinary team to support emotional well-being.

The role operates Monday–Friday, 8 am–4:30 pm with a Conway coverage area, offering a comprehensive benefits package and opportunities for growth within a mission-driven hospice provider.

Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed as a Social Worker in the state.
• Minimum 1 year hospice or healthcare social work experience preferred.

Responsibilities

• Assess psychosocial status and communicate findings to the RN Case Manager and interdisciplinary team.
• Evaluate medical needs, home situations, resources, and community support for patients.
• Develop and implement personalized social work plans and coordinate with Bereavement Coordinator.
• Attend interdisciplinary meetings and support care plan development.
• Provide empathetic counseling to patients and families.
