Job summary

Crescent Hospice is seeking a Licensed Master Social Worker to provide compassionate psychosocial care to hospice patients and their families. You will assess needs, coordinate with the care team, and offer counseling to support emotional well-being during end-of-life care.

Schedule is Monday–Friday, 8 am–4:30 pm in Beaufort, Jasper, Hampton, and Colleton counties. A competitive benefits package and professional growth opportunities are offered to eligible staff.

Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed Social Worker in SC with active license.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and interdisciplinary team.
• Evaluate home situations, resources, and community support for patients/families.
• Develop individualized social work plans and coordinate with Bereavement Coordinator.
• Participate in care planning and attend interdisciplinary meetings to support caregivers.
• Provide empathetic counseling to patients and families.
