Job summary

Crescent Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) to deliver compassionate social work services to our hospice patients and families in South Carolina. This per diem role focuses on counseling, planning, and supporting emotional well-being during challenging times.

As an MSW with us, you will assess psychosocial needs, collaborate with the RN Case Manager and interdisciplinary team, and advocate for each patient’s dignity and comfort while guiding families through

Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed as a Social Worker with the State Board.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle.
• Hospice experience: 1 year preferred; healthcare setting experience required.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and team.
• Evaluate home situations, resources, and community support for patients.
• Develop and implement individualized social work plans with Bereavement collaboration.
• Participate in interdisciplinary meetings and support family counseling.
• Provide empathetic counseling to patients and families.
