Job summary

Athena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our team. You will plan, organize, and direct the hospice care plan for assigned patients under the clinical director, with travel throughout Rhode Island.

We offer $45–$50 hourly, weekly pay, and benefits. Active Rhode Island RN licensure and a nursing degree are required; hospice experience is preferred. This role emphasizes compassionate, professional nursing care for patients and families.

Qualifications

• Active Rhode Island RN license
• Nursing degree from an accredited program
• Hospice experience preferred (not required)

Responsibilities

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
• Provide professional nursing care by utilizing all elements of the nursing process
• Supervise hospice aides and coordinate care with the clinical team
• Develop and implement a care plan based on nursing diagnosis and patient goals
• Provide counseling to patients and families in meeting nursing and related needs
