Job summary

Athena Hospice of Rhode Island is seeking a Part-time RN Case Manager to coordinate and deliver compassionate hospice care for patients statewide. This on-the-road role offers mileage reimbursement and flexible scheduling to meet patient needs.

You will assess patients, develop care plans, supervise aides, and collaborate with families and clinicians. Requires Rhode Island RN license and nursing degree; prior hospice experience preferred.

Qualifications

• Active RN license in Rhode Island.
• Nursing degree from an accredited program.
• Prior hospice experience preferred.

Responsibilities

• Plan, organize, and direct hospice care for assigned patients.
• Conduct comprehensive patient assessments and develop care plans.
• Coordinate with families and healthcare professionals to implement plans.
• Supervise hospice aides and provide guidance on patient care.
• Provide emotional and spiritual support to patients and families.
