Job summary

Athena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our compassionate team. You will plan, organize, and direct the hospice care plan for assigned patients under the clinical director, with a competitive hourly rate of $45 to $50 and weekly pay, and travel throughout Rhode Island required.

This role requires an active Rhode Island RN license and a nursing degree; prior hospice experience is preferred.

Qualifications

• Active and unencumbered Rhode Island RN license.
• Nursing degree from an accredited college, university, or school.
• Prior hospice experience preferred.

Responsibilities

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
• Provide a complete physical assessment and history of current and previous illnesses
• Supervise hospice aides
• Provide professional nursing care by utilizing all elements of the nursing process
• Assess and evaluate patient status, writing and initiating a plan of care, and regularly re-evaluating patient and family/caregiver needs
• Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions
• Provide counseling to patients and families in meeting nursing and related needs
