Job summary

Holy Cross Home Care and Hospice (Maryland) is seeking a Hospice Medical Social Worker (MSW) to provide compassionate, patient-centered psychosocial support in patients' homes across Prince George's County. You will assess psychosocial needs, connect families with community resources, support care planning, and advocate for goals that promote dignity, stability and independence.

• Master's Degree from a school of social work accredited by the CSWE
• State of Maryland Social Work License Required
• Two (2) years of social work experience in health care setting, preferably hospice
• Ability to provide service to acutely and chronically ill patients
• Ability to complete written, verbal, and observational tasks

• Pay Range $30.31-$48.50 per hour (based on years of experience, skillset, etc.)

Minimum Qualifications

• Master's Degree from a school of social work accredited by the Council on Social Work Education

• State of Maryland Social Work License Required,

• Two (2) years of social work experience in health care setting, preferrable hospice experience.

• Ability to provide service to acutely and chronically ill patients

• Ability to complete written, verbal, and observational tasks indigenous to the position.
