Job summary

Holy Cross Home Care and Hospice in Maryland seeks a Hospice Medical Social Worker (MSW) to deliver patient‑centered psychosocial support in home settings, assess needs and connect patients with resources to support care planning.

You will advocate for goals that promote dignity, stability and independence, with autonomy in a meaningful, 1:1 patient relationship.

Qualifications

• Master's Degree from a school of social work accredited by the CSWE.
• Maryland Social Work License required.
• Two years of social work experience in healthcare; hospice preferred.
• Ability to serve acutely and chronically ill patients.
• Strong written, verbal and observational communication.

Responsibilities

• Provide psychosocial support in patients' homes and assess needs.

Minimum Qualifications

• Master's Degree from a school of social work accredited by the Council on Social Work Education

• State of Maryland Social Work License Required,

• Two (2) years of social work experience in health care setting, preferrable hospice experience.

• Ability to provide service to acutely and chronically ill patients

• Ability to complete written, verbal, and observational tasks indigenous to the position.
