Qualifications

• Maryland Social Work license required.
• Two years of social work experience in a healthcare setting, hospice preferred.
• Ability to deliver service to acutely and chronically ill patients.

Responsibilities

• Provide compassionate, patient-centered social work support in patients' homes.
• Assess psychosocial needs, connect patients with community resources, and support care planning.
• Advocate for goals that promote dignity, stability and independence.
