Coastal Hospice in Maryland seeks an experienced RN to provide comprehensive hospice care across inpatient and home settings. You will develop and evaluate care plans, administer medications, and collaborate with families and medical teams to ensure high-quality patient outcomes.

Applicants must hold current MD/DE (or compact state) RN licensure, with BSN preferred and CHPN certification expected within two years. Lifting up to 50 pounds and on-call rotations may be required.

Qualificações

• Licensed and registered RN in MD, DE, or compact state.
• BSN from an accredited college preferred.
• CHPN certification expected within 2 years.

Responsabilidades

• Develop, implement, and evaluate patient care plans per ANA standards and nursing process.
• Educate patients and families on significant self-care and end-of-life issues and hospice services.
• Triage phone calls to identify patient/family needs and notify Admission Services of referrals.
• Oversee and delegate assignments of CNAs, LPNs, Hospice Aides, students and volunteers on shift.
• Coordinate discharge planning with family and medical provider.
• Maintain licenses and continuing education obligations.
