Job summary

Coastal Hospice invites you to join a team of caregivers dedicated to promoting dignity and quality of life for patients and families facing life-limiting conditions in Salisbury, MD. As an RN Case Manager, you will develop and evaluate care plans, oversee medications, and educate families to ensure compassionate, patient-centered hospice care at home.

Qualifications

• Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state.
• BSN from an accredited college preferred.
• CHPN certification expected within two (2) years of employment.
• Ability to lift and carry up to fifty (50) pounds with or without assistive devices.

Responsibilities

• Develop, implement and evaluate a plan of patient care utilizing ANA Standards and Nursing Process.
• Oversees and ensures safe handling of medications and nursing procedures.
• Educates patients and families on hospice services and end-of-life issues.
• Acts as a patient advocate and provides support, information and direct care.
• Triage phone calls to identify patient/family needs and notify Admissions for referrals.
• Oversees and delegates tasks to CMTs, LPNs, Hospice Aides, students and volunteers on shift.
• Participates in weekend, holiday, on-call and evening/night rotations.
• Maintains licenses and continuing education obligations.
• Cover different areas of hospice care to meet organizational needs.
• Provide medical care to a manageable caseload in home settings.
• Coordinate care with Interdisciplinary Team to align with patient/family goals.

RN Case Manager

Coastal Hospice invites you to join a team of caregivers who are committed to promoting dignity and quality of life for patients and families who face life-limiting conditions. In your role as a RN Case Manager, you will help ensure that every patient and family is honored, supported and comforted as they receive the right care at the right time throughout their journey by:

• Responsible for developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
• Utilizes equipment and technology skillfully and is technically competent in nursing procedures
• Oversees and/or participates in safe medication handling and practices
• Educates patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensures consent and payer forms are completed
• Acts as a patient advocate. Provides support, information and direct care
• Triage phone calls to identify patient/family needs and notifies Admission Services of referrals
• Oversees and delegates assignments of CMTs, LPNs, Hospice Aides, students and volunteers on shift
• Participates in weekend, holiday, on call and/or evening/night rotations
• Maintains appropriate licenses and adheres to continuing education obligations
• Ability to cover different areas of hospice care to assure organizational needs are met
• Provides medical care to a manageable caseload of hospice patients in their home settings
• Develops plan of care alongside an Interdisciplinary Team in line with the patient/family needs and goals
• Coordinates all aspects of home hospice care including symptom management, spiritual, psychosocial, financial, and bereavement needs
• Works closely with the Interdisciplinary Team to assure patient/family goals are met

Qualifications

Education and Work Experience Requirements

• Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
• BSN from an accredited college preferred
• CHPN certification expected within two (2) years of employment
