Better Together Hospice, Akron, OH, is seeking a Part-Time Hospice Chaplain to provide spiritual and emotional support to patients and families during end-of-life care. The role involves assessing spiritual needs, developing care plans, and offering prayer and active listening, with travel to patient homes and contracted facilities within the service area.

• Strong communication, listening, interpersonal and documentation skills.
• Compassionate, nonjudgmental approach to individuals from diverse faiths and beliefs.
• Ability to work independently and collaboratively with an interdisciplinary team.

Responsabilités

• Provide spiritual and emotional support to hospice patients and their families.
• Assess individual spiritual needs, beliefs, and preferences.
• Develop personalized spiritual care plans based on patient and family needs.
• Offer prayer, scripture, mediation, and other spiritual support as requested.
• Provide emotional support for fear, anxiety, or end-of-life concerns.
• Offer bereavement support and guidance to family members as appropriate.
• Collaborate with nurses, social workers, aides, and other hospice care team members.
• Communicate changes in patient or family needs to the care team.
