Job summary

Qualifications

• Must have completed a minimum of 75 hours of classroom and supervised practical training (16 hours classroom before 16 hours practical).
• At least 18 years of age.
• Ability to read and follow written instructions and document care; understands hospice philosophy.
• Satisfactory references from previous employers.
• Is self-directing with the ability to work with little direct supervision.
• Shows empathy for the needs of the ill and dying.
• Demonstrates tact, patience and good personal hygiene.
• Licensed driver with an insured automobile.
• Successfully completes a probationary period to ensure competency.

Responsibilities

• Perform personal care tasks after assignment and promptly attend to requests.
• Provide positive communication with patient and family, maintaining confidentiality.
• Ensure safety and proper use of equipment (foot stools, side rails, O2, etc.).
• Provide personal care including baths, back rubs, oral hygiene, shampoos and linen changes.
• Assist with dressing and undressing as assigned.
• Plan and prepare nutritious meals, including shopping when needed.
• Assist in feeding the patient as assigned.
• Observe and report on patient condition and skin integrity to prevent tissue breakdown.
• Support mobility and transfers as directed by the nurse.
