Job summary

Soleo Health is seeking a Part-time Infusion Nurse to support our infusion patients in Cypress, TX and surrounding areas. The role provides primary patient care, including assessments, care planning, and education for patients receiving home infusion therapy in accordance with established care matrices.

The ideal candidate is an RN licensed in Texas, with infusion experience and the ability to participate in an on-call rotation, with mileage reimbursement and a patient-centered approach.

Qualifications

• Current RN license by the state Board of Nursing.
• RN degree or BSN preferred.
• Administrative oversight responsibilities.
• Participation in an on-call rotation, including nights and weekends.
• Minimum 3 years nursing experience with infusion therapy preferred.

Responsibilities

• Provide direct patient care for all assigned patients and coordinate with care team members.
• Educate patient and care partner on safe home infusion therapy and procedures.
• Assess home environment and implement safety interventions for prescribed care.
• Develop and evaluate the plan of care with the team.
• Manage schedule via company platform and communicate changes to the team.
• Coordinate patient care via approved company platform.
• Participate in nurse on-call rotation to support continuous nursing services.
