Job summary

Soleo Health is seeking a Part-time Infusion Nurse to support infusion patients in Windsor, CO and surrounding area. The role provides primary patient care including assessments, care planning, and education in the home or alternate sites.

You will collaborate with pharmacists, prescribers, dietitians, and care partners, conduct home safety evaluations, and ensure timely documentation and adherence to policies.

Qualifications

• Current RN license by state board in good standing.
• BSN degree from an accredited institution preferred.
• Advanced certifications (CRNI, ONS, IgRN) preferred but not required.
• Minimum 3 years nursing infusion experience preferred.
• Experience in ICU and/or cardiac care preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite.
• Valid driver's license with no significant restrictions.

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with appropriate care team members.
• Evaluate and educate patient/care partner to safely administer home infusion therapy and provide additional instruction as indicated.
• Evaluate the home environment where care is provided and identify interventions for safety and appropriateness.
• Develop with team members the plan of care and evaluate response to therapy.
• Manage schedule via company platform and communicate changes.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation for continuous nursing support.
• Document all patient encounters and care activities per policy and regulations.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of POTs and required clinical documents.
• Work with departments to ensure required patient documents are completed and returned.
