Job summary

Canyon Idaho Home Care and Hospice is seeking a Speech-Language Pathologist to assist in evaluating patients, developing and revising care plans, and documenting clinical progress. The role requires licensure in the service state and an advanced degree in speech-language pathology.

Responsibilities include coordinating with physicians and family, participating in staff training, and ensuring adherence to agency policies. Prior experience is preferred, with a patient-centered approach to therapy.

Qualifications

• Must hold a master’s or doctoral degree in speech-language pathology.
• Must be licensed as a speech-language pathologist in the state where services are provided, or meet provincial/national licensing requirements as applicable.
• Two years of experience preferred.

Responsibilities

• Assists physician or intermediate care provider in evaluating function and developing the care plan.
• Prepares clinical and progress notes.
• Advises and consults with family and other agency personnel.
• Participates in in-service programs and presents as assigned.
• Adheres to established policies and procedures.
• Collaborates with all health care personnel in patient care planning.
