Job summary

ABL Health Care in Concord, CA, seeks a compassionate Registered Nurse (RN) for Home Health on a part-time, per diem basis. You will evaluate patients, implement care plans, and track progress alongside our Clinical Director and field staff.

You must hold a California RN license and have at least 1 year of Home Health or Hospice experience. A valid CA driver's license and ability to travel 15–25 miles are required.

Qualifications

• Current RN license to practice in California.
• At least 1 year of Home Health or Hospice experience.
• Valid California driver’s license.
• Willingness to travel within a 15–25 mile radius.

Responsibilities

• Provide nursing care within the scope of practice per the patient care plan.
• Administer treatments, monitor conditions, and report changes to physicians and care team.
• Maintain accurate clinical records using company devices.
• Supervise LVNs when assigned.
• Comply with all policies to ensure high standards of care.
