Job summary

Amedisys Home Health is seeking a registered nurse case manager to support Havens area patients on a part-time basis, 20-30 hours weekly. You will assess patients, develop care plans with the care team and supervise nursing staff while maintaining thorough documentation.

This role emphasizes teaching, care coordination, and on-call duties within the home-health setting. Eligible candidates bring RN licensure in the state, CPR certification, and a valid driver’s license.

Qualifications

• 1+ year of clinical RN experience; approval by VP Clinical if less.
• Current RN license for assigned states.
• CPR certification is required.
• Valid driver's license and transportation.

Responsibilities

• Perform patient assessments with the care team to develop a care plan.
• Make referrals to other disciplines as needed with rationale.
• Educate patients to promote independence and self-care.
• Supervise LPNs and HHAs.
• Complete timely, accurate documentation.
• Regularly report patient progress to the clinical manager.
• Plan staff education and participate in on-call responsibilities.
• Engage in clinical development and continuing education.
