Amedisys is seeking a Registered Nurse Case Manager to join our home health care team. You will perform patient assessments, develop and implement care plans, and collaborate with the care team to promote patient health at home.

Responsibilities include supervising LPNs and HHAs, documenting progress, and participating in ongoing professional development. Valid RN license and CPR certification required.

Formación

• RN with at least 1 year of clinical experience.
• Active RN license in the state(s) you are assigned to work.
• CPR certification required.

Responsabilidades

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patients needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services as assigned.
• Participates in clinical development and continuing education programs.
