Job summary

HomeChoice Health Services, part of the LHC Group and Optum family, is seeking a Registered Nurse in Home Health to provide and direct nursing care in patient homes. You will coordinate total plan of care with physicians and the health care team, ensuring compliance with laws, regulations, and agency policies.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license with reliable transportation.
• CPR certification preferred or to be obtained within 90 days of hire.
• 1+ years of home health experience preferred.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to state and federal regulations.
• Completes OASIS assessments accurately and timely, including medication reconciliation.
• Coordinates care and communicates with patients, families, and clinicians to ensure quality care and service excellence.
