Job summary

Suncrest Home Health Southside, LLC in Atlanta, GA is seeking a part-time Physical Therapist to provide in-home care and coordinate with physicians for consistent treatment. The role requires GA PT licensure, CPR/First Aid, and 2+ years of PT experience.

The therapist will develop individualized treatment plans, educate patients and families, and maintain progress notes, while adhering to regulations and continuing education requirements.

Qualifications

• Valid Georgia Physical Therapist license.
• CPR and First Aid certification required.
• Minimum two years of PT experience preferred.
• Must have medical licenses and certifications for the locality.
• Experience in an Integrated Healthcare Delivery System is a plus.
• Excellent work ethic and dedication to patient success.

Responsibilities

• Meet with new patients and give tours of the facility.
• Devise detailed treatment and care plans for each patient.
• Communicate with patients’ physicians to ensure consistent care.
• Visit patients in their homes and other facilities on a limited basis.
• Implement effective treatment combinations using exercises, stretching, equipment and hands-on therapy.
• Evaluate and report on patients’ progress.
• Assist administrative staff with billing and reporting inquiries.
• Record progress and adjust plans as needed.
• Educate patients and families about the recovery process.
• Performs related work as required.
