Physical Therapist - Suncrest Home Health - Part Time

Suncrest Home Health Southside, LLC Full Time Non Exempt Atlanta, GA, US

Duties/Responsibilities:

• Meet with new patients and give them tours of our facility
• Devise detailed treatment and care plans for each of the patients
• Communicate and coordinate with patients’ physicians to ensure a consistent level of care
• Visit patients in their homes and at other facilities on a limited basis
• Exercise independent judgment based on experience to implement effective treatment combinations
• Evaluate and report on patients’ progress
• Assist administrative staff with billing and reporting inquiries
• Comply with federal and state physical therapy regulations
• Complete at least 10 hours of continuing education every two years
• Reviewing patients’ medical histories
• Diagnosing patients by observing their movements and listening to their concerns
• Developing individualized treatment plans for patients
• Outlining clear goals for patients and the expected outcomes of the plan
• Using exercises, stretching, equipment, and hands-on therapy to manage patients’ pain, increase mobility, and prevent further pain and injury
• Recording patients’ progress and modifying the plan of care if needed
• Educating patients and family members about the recovery process
• Performs related work as required

• A degree in medicine is a basic prerequisite
• Must have completed family medicine internship and residency
• Must possess all the necessary medical licenses and certification needed to work in a particular locality.
• Experience working in an Integrated Healthcare Delivery System is a plus

Education and Experience:

• Valid state of Georgia Physical Therapist license
• Excellent work ethic and dedication to patient success
• Basic computer skills
• CPR and first aid certification required
• A minimum of two years of experience working in the physical therapy field preferred.
