Job summary

AccentCare, Inc. is seeking a Physical Therapist for Home Health in Georgia. The role covers a defined service area with full-time employment and a salary range of $99,000 to $110,000 based on experience.

The candidate will evaluate and treat patients, educate families, and collaborate with an interdisciplinary team. Requirements include a CAPTE-accredited Master’s in Physical Therapy, at least 9 months of PT experience (home health preferred), current state license, CPR certification, and

Qualifications

• Master’s degree in physical therapy approved by CAPTE accredited program.
• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Currently licensed to practice as a physical therapist within the state of operation.
• Must possess and maintain CPR certification.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provides patient care and documents treatment as ordered, per agency policy and patient needs.
• Performs patient evaluations and selects appropriate diagnostic material.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with measurable goals.
• Educates and counsels patients, coordinating with the care team.
• Presents patient information to physicians and keeps team informed of status and plan.
• Delivers appropriate treatment within agency and payer guidelines.
• Meets productivity standards and utilizes work hours efficiently.

Skills

9 months PT experience

Home health experience preferred

CPR certification

Licensed PT in state of operation

Education
