Qualifications

• Minimum one (1) year of healthcare experience preferred.
• Home Health experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform physical therapy evaluations and ongoing patient assessments.
• Develop and implement individualized therapy treatment plans.
• Monitor patient progress and communicate changes to physicians.
