Job summary

Provide personalized rehabilitation at patients' homes, focusing on mobility, balance, and independence, while collaborating with the clinical team and maintaining timely documentation.

Qualifications

• Active Massachusetts PT or PTA license in good standing.
• Strong communication and clinical skills.
• Ability to work independently in a home health setting.
• Reliable transportation.
• Previous home health experience preferred.

Responsibilities

• Provide individualized physical therapy services during scheduled home visits.
• Help patients improve strength, mobility, balance, and functional independence.
• Assist patients recovering from surgery, hospitalization, injury, or illness.
• Provide therapeutic exercises, gait training, and fall-prevention education.
• Educate patients and caregivers on safe mobility and home exercise programs.
• Monitor patient progress and communicate with the interdisciplinary clinical team.
