Job summary

Beth Israel Lahey Health at Home in Beverly, MA is seeking a Physical Therapist for home health with a 32-hour work week. You will plan, organize, and direct home care services, coordinating care for an assigned caseload and supervising PTA staff as needed.

The role requires a PT degree, MA license, and BLS; 2+ years in PT with home health experience is preferred. Expect HIPAA-compliant documentation and strong OASIS proficiency in a supportive team environment.

Qualifications

• Degree from an accredited Physical Therapy program.
• Massachusetts Physical Therapist license active.
• BLS certification (AHA).
• 2+ years PT experience, at least 1 in home care/public health.
• OASIS experience preferred.

Responsibilities

• Coordinate patient care caseload including supervising PTA staff when applicable.
• Complete initial patient and family assessment to plan home care.
• Develop patient-specific plan of care with goals and family involvement.
• Initiate and revise the plan as patient status changes.
• Initiate appropriate preventive and rehabilitative therapy procedures.
• Administer therapies and treatments per physician orders.
• Communicate with physicians and document OASIS timely.
• Maintain HIPAA confidentiality.
