Job summary

Beth Israel Lahey Health is seeking a Physical Therapist to deliver home health services across Peabody and Lynnfield, planning, coordinating, and directing care in patients' homes and communities. The role involves collaborating with families to build a patient-specific plan of care and meeting home care regulations.

The ideal candidate holds MA licensure, a DPT is preferred, and has 2+ years of PT experience with OASIS familiarity.

Qualifications

• Degree from an accredited program in Physical Therapy with MA licensure.
• DPT preferred; MA license active and in good standing.
• BLS certification is required.
• At least 2+ years in physical therapy, including home health or public health.

Responsibilities

• Coordinate care for assigned patient caseload, supervise PTA staff when applicable.
• Complete initial patient assessment and develop plan of care with patient/family involvement.
• Initiate and revise plan of care; deliver therapeutic, preventive, and rehabilitative services.
• Communicate with physicians for orders and changes in patient condition; maintain HIPAA compliance.
• Demonstrate proficiency with OASIS and timely documentation.
