The New Jewish Home CHHA is seeking a part-time Physical Therapist to provide and coordinate multidisciplinary home-based care and rehabilitation therapy for patients recovering from health events. The role collaborates with physicians and the care team to implement the plan of care.

The incumbent will perform OASIS assessments, supervise aides and PT assistants, and ensure timely documentation and reporting of patient progress. Clinical coordination and team conferences are required.

Responsabilités

• Collaborates with physicians, patients and families to develop and implement patient-specific care and treatment plans.
• Educates patients to promote health and independence.
• Assesses, monitors, documents and reports patient progress; supervises aides and PT assistants.
• Observes signs and symptoms and reports changes to physician or staff.
• Completes OASIS assessments timely and documents patient health status.
• Supervises occupational therapy assistant according to state regulations.
• Coordinates total plan of care and liaises with staff; participates in team/case conferences for complex cases.
• Performs other duties as assigned.
