Job summary

The New Jewish Home in New York City seeks a CHHA Fee For Service Registered Nurse to provide and coordinate multidisciplinary health services for patients in the community. Weekend hours are available and training occurs during the week.

You will conduct assessments and collaborate with physicians and other team members on care plans, aiming for discharge from the program while ensuring documentation stays within regulatory guidelines.

Job description

The New Jewish Home Certified Home Health Agency (CHHA) provides home based care and rehabilitation therapy for patients recovering from an acute episode or other adverse health event. The New Jewish Home has long experience delivering rehabilitation and home care services. Our goal is to help each patient get well, stay well and return to health and independence as quickly as possible.

We are currently looking for a CHHA Fee For Service Nurse who can cover the Manhattan Area. Weekend hours also available. Training during the week.

Job Summary:

TheCHHA Fee For Service Registered Nursewill provide and coordinate multidisciplinary health services for patients in the community Conduct assessment and collaboratewith the Physician and other team members on the implementation ofcare planwith a goalfor client to bedischarge from program.

Further duties includeensuring thatall documentation requiredare completed within regulatory guidelines includingPhysician Orders (485) and Oasis which must accurate reflect clinical and functional status of patient in order to follow reimbursement policies.
