Responsabilités

• Evaluates clients to obtain and interpret data for treatment planning in the home setting.
• Performs initial assessment/start of care including OASIS when applicable.
• Develops an individualized plan of care with physician collaboration.
• Communicates with physician to revise the plan of care as needed.
• Establishes patient-centered goals and collaborates with the interdisciplinary team.

Licensed in Iowa to practice physical therapy
