BAYADA Home Health Care is seeking a Part-Time Home Health Occupational Therapist to perform home visits for adult and geriatric patients in Westford, Groton, Littleton, Acton and surrounding towns. You will join a multidisciplinary team delivering rehabilitative care and skilled nursing in patients' homes.

A MA OT license and at least 1 year of OT experience are required; prior home health experience is not required, and a flexible schedule is offered.

Formación

• Massachusetts Occupational Therapist license is required.
• Minimum of one year of recent work experience as an Occupational Therapist.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop individualized intervention plans.
• Evaluate home environment for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations for better outcomes.
• Continually assess and revise care plans with nursing as part of a multidisciplinary team.
• Educate patients and families in rehabilitative care and activities to promote health and independent living.
• Document observations and evaluations accurately using a touch pad tablet.
