BAYADA Home Health Care is seeking a part-time or full-time Occupational Therapist to provide home health visits for adult and geriatric patients in Dartmouth, Westport, Fall River, Swansea, Seekonk and surrounding towns. Prior home health experience is not required.
The OT will join a multidisciplinary team delivering rehabilitative care in patients’ homes, educating families, and updating care plans as needed.
Competenze
- Massachusetts OT license is required.
- Minimum of one year of recent work experience as an Occupational Therapist.
Mansioni
- Make home visits to clients in designated geographic territories.
- Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
- Assess cognitive, psychological and motor aspects to develop intervention plans.
- Evaluate home environment for hazards or barriers to independence.
- Identify equipment needs and environmental adaptations.
- Continually revise care plans and collaborate with nursing in the multidisciplinary team.
- Educate patients and families in rehabilitative care and activities.
- Document observations and interventions on a touch pad tablet.
