Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist for a part-time role providing home health visits to adults and geriatric patients in Dartmouth, Westport, Fall River, and surrounding towns.

Responsibilities include evaluating functional status, developing individualized plans, ensuring home safety, and educating clients to promote independence, with documentation via tablet. Prior home health experience is not required but preferred.

Qualifications

• Massachusetts OT license is required.
• Minimum of one year of recent OT experience.
• Home health experience preferred but not required.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status, including ADLs and IADLs.
• Assess cognitive, psychological and motor aspects of performance and develop plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• continually assess and revise the OT care plan and collaborate with nursing.
• Educate patients and family members in rehabilitative care to promote health and independence.
• Document observations and interventions accurately using a touch pad tablet.
