Job summary

ABL Health Care, LLC is seeking a skilled Occupational Therapist to provide intermittent home health services in Fremont, Newark, and Union City. This part-time per diem role involves evaluating patients, implementing and revising care plans, and coordinating with the Clinical Director.

Candidates must hold a California OT license, have at least 1 year of home health or hospice experience, and be able to travel within a 15–25 mile radius. A California driver’s license is required.

Qualifications

• Current California OT license to practice.
• Experience in Home Health or Hospice care – 1 year required.
• Current California Driver's License.
• Able to travel within a 15–25 mile radius regularly.

Responsibilities

• Provide, coordinate, and oversee skilled intermittent care to patients in their homes, including instruction and education of patients and/or caregivers to promote maximal functional status.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change to Physician, Case Manager, or Director of Nursing.
• Initiate, develop, implement, and revise the patient's plan of care (POC).
• Address all problems identified in the POC.
• Provide educational information to the patient/family/caregiver regarding knowledge deficits or needs about the patient's condition.
• Communicate with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation per company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.
