Job summary

ABL Health Care LLC is seeking an experienced, self-driven Occupational Therapist (OT) for Home Health in Fremont. The full-time role involves evaluating patients, creating and implementing care plans, and tracking progress to uphold high-quality care in the field.

The OT will work with the Clinical Director and team, traveling within the region and providing remote and in-field support to patients and families. CA licensure and a valid driver’s license are required.

Qualifications

• Current OT license to practice in California (CA)
• Experience in Home Health or Hospice care (1 year)
• CA Driver's License
• Able to travel within a 15–25 mile radius regularly

Responsibilities

• Provide, coordinate and oversee skilled intermittent care in the patient’s home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments and monitor patient conditions; report changes to relevant team members.
• Develop and revise the patient’s plan of care (POC) as needed.
• Educate patient/family/caregiver on condition and care needs.
• Maintain accurate clinical records and documentation on company device.
