Job summary

YMCA of Pierce and Kitsap Counties seeks a Referral Coordinator to support Health Initiative services. You will reach out by phone and email to assist referred individuals with enrollment, answer questions, and connect to chronic disease programs, YMCA membership, and other resources.

Responsibilities include maintaining health data, coordinating care conversations, and supporting enrollment targets while providing strong customer service. Equal opportunity employer.

Qualifications

• Experience or education in health promotion/public health fields.
• Care management and motivational interviewing skills preferred.
• Comfort with health information and privacy policies.

Responsibilities

• Maintain electronic health data and referral records per privacy rules.
• Conduct care coordination conversations using motivational interviewing.
• Connect individuals to YMCA services and external resources.
• Support enrollment targets and program reporting.
• Assist with marketing and class enrollment.
• Ensure correct intake forms and documentation.
