Job summary

Pioneer Human Services in Washington state seeks a part-time Health Coordinator to coordinate and monitor client physical and behavioral health services within residential facilities. You will facilitate communication among clients, nursing staff, medical and behavioral health providers, and program staff.

The role includes intake assistance, coordinating medical appointments (including telehealth), medication monitoring documentation, and provider order implementation under nursing direction

Qualifications

• High School Diploma or Equivalent.
• One year of experience in caregiving or human services.
• Must apply for Agency Affiliated Counselor registration within 30 days and obtain within 90 days.
• First Aid/CPR certification within 90 days of hire.
• Strong written and verbal communication with attention to detail.
• Basic computer skills and ability to learn.
• Empathetic, client-centered approach to recovery.

Responsibilities

• Coordinate and monitor client physical and behavioral health services.
• Assist intake, schedule appointments, including telehealth coordination.
• Document in electronic health record and maintain records accurately.
• Provide direct client support in a structured residential setting.
• Flag safety concerns and liaise with nursing staff as needed.
