Job summary

Trinity Health PACE seeks a part-time physician to provide primary medical care for PACE participants at Mercy LIFE Assisi House (Aston) and Mercy LIFE Valley View (Elwyn). Two 8-hour days at Assisi House and one 4-hour day at Valley View, with flexible scheduling and no night/weekend calls.

The PCP will work with the interdisciplinary team to assess, plan, and manage care, learn ASL, and support a person-centered care approach in a aging and deaf community.

Qualifications

• MD or DO with active license to practice in state of PO.
• CDS certification and DEA registration required.
• BLS certification required.
• Board certification Internal Medicine or Family Practice; geriatrics preferred.
• Willingness to learn American Sign Language.
• Minimum one year experience with frail or elderly population.
• Valid driver’s license and use of personal vehicle.
• Strong communication, problem solving, and organizational skills.
• Proficiency with Microsoft Office.
• Ability to adapt to changing priorities.
• Travel within the Center's geographic region.

Responsibilities

• Participate as a member of the interdisciplinary team and report changes to staff daily.
• Provide initial and ongoing assessments and care plans integrated into the overall plan.
• Refer participants to medical specialists; manage care in the nursing home.
• Oversee use of specialists and inpatient care; follow up on outcomes.
• Review medical outcomes to establish best practices and document with codes.
• Advise Medical Director on accountability and resource needs as programs expand.
• Assist with policy/procedure development; train staff on medical issues.
• Participate in provider on-call rotation as required.
