Job summary

Trinity Health PACE is seeking a Part-time Primary Care physician to provide medical care to PACE participants at Mercy LIFE Assisi House, Aston PA, and Mercy LIFE Valley View, Elwyn PA. You will perform assessments, develop care plans with the interdisciplinary team, and manage routine medical care.

This role requires two 8-hour days at Assisi House and one 4-hour day at Valley View, with flexible days and no night or weekend coverage.

Qualifications

• Doctorate degree (MD or DO) and active license in PA.
• Board certification in Internal Medicine or Family Practice.
• Geriatrics board certification preferred.
• Willingness to learn American Sign Language.
• Experience with frail or elderly populations desirable.
• Strong communication and organizational skills.

Responsibilities

• Participate as a member of the interdisciplinary team and conduct assessments.
• Develop and implement care plans for PACE participants.
• Refer to specialists and manage care with nursing home collaboration.
• Ensure documentation and diagnostic coding are current.
• Advise on program resources and policy development.
• Participate in provider on‑call rotation as required.
