Qualifications

• Excellent oral and written communication and interpersonal skills.
• Adequate computer skills to complete job duties and documentation.
• Demonstrated ability to assess patient needs and develop a comprehensive plan of care.
• Knowledge of accreditation standards and compliance requirements.

Responsibilities

• Review patient information and assess impact on evaluation and treatment.
• Conduct functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with functional goals.
• Comply with evaluation, treatment and documentation guidelines.
• Record patient progress at least every seven days.
• Revise treatment plans as needed during the course of treatment.
• Document the course of treatment clearly and discharge summaries.
