Job summary

Blueprint Healthcare is seeking a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide part-time primary care to residents in assisted living facilities across Rhode Island.

This role emphasizes proactive, preventive care and value-based outcomes, with on-site visits, annual wellness exams, chronic disease management, and close coordination with facility staff to ensure seamless, patient-centered care.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Board certification (AANP, ANCC, or NCCPA).
• Active DEA license (or ability to obtain).
• Experience in geriatrics, primary care, assisted living, or SNF preferred.
• Strong clinical decision-making and diagnostic skills.

Responsibilities

• Provide on-site primary care to residents in assisted living facilities.
• Conduct routine visits, assessments, and annual wellness exams.
• Evaluate and treat acute and chronic medical conditions.
• Develop, implement, and monitor individualized care plans.
• Order and interpret labs, diagnostic tests, and imaging.
• Coordinate with facility staff, nurses, and caregivers to ensure continuity of care.
• Maintain accurate EMR documentation and ensure regulatory compliance.
