Job summary

Qualifications

• Graduate of an accredited NP or PA program.
• Board certification in the respective field.
• Active DEA license or ability to obtain.

Responsibilities

• Provide on-site primary care to residents in assisted living facilities.
• Perform annual wellness exams and routine visits.
• Assess and manage acute and chronic conditions with individualized care plans.
• Order and interpret labs, imaging, and tests.
• Collaborate with facility staff to ensure continuity of care.
