• Conduct psychosocial assessments for residents on admission and as needed
• Develop, implement, and monitor individualized treatment plans
• Provide individual, group, and family therapy sessions
• Address depression, anxiety, dementia, and adjustment to illness or disability
• Collaborate with interdisciplinary team to coordinate care
• Maintain HIPAA-compliant documentation and ethical standards
• Provide crisis intervention and behavioral management as needed
• Educate staff and families on mental health and effective communication
