Vitae Health Systems is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes.

The position offers a part-time schedule (2 days per week) with a base compensation range of $35,000–$55,000 and potential additional earnings through commission-based incentives.

Qualifications

• Master's degree in Psychology, Counseling, Social Work, or related field.
• Active, unrestricted state license (LCSW/LPC/LMFT or Licensed Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric/long-term care.
• Strong clinical assessment, diagnostic and treatment planning skills.
• Knowledge of SNF mental health conditions (depression, dementia, etc).
• Proficiency in CBT, trauma-informed care, grief counseling.
• Excellent communication and collaboration in interdisciplinary teams.

Responsabilités

• Conduct psychosocial assessments on admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness.
• Support residents coping with chronic illness, loss of independence, and end-of-life concerns.
• Collaborate with the Vitae clinical team, attending physicians, nursing staff, social workers, and rehab teams.
• Participate in care plan meetings and interdisciplinary discussions.
• Maintain accurate clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management when needed.
• Educate staff and families on mental health and effective communication.
• Ensure HIPAA compliance and ethical standards.
