Job summary

Vitae Health Systems is seeking licensed psychotherapists to provide mental health services to residents in skilled nursing facilities in the Chicago area. Opportunities include both full-time and part-time roles, with 1-3 days per week availability for part-time positions.

You will assess, diagnose, and treat emotional and behavioral conditions, develop treatment plans, and collaborate with an interdisciplinary team to support residents' well-being and adjust to medical life changes.

Qualifications

• Master's degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• 2+ years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Collaborate with physicians, nursing staff, social workers, and rehabilitation teams to coordinate care.
• Maintain accurate clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management strategies when needed.
