Job summary

Associated Home Care is seeking an RN for part-time, in-home care to administer the care plan and monitor patient health per regulations. The role requires coordination with the nursing director and care team, and involves supervising home care aides and maintaining compliance with standards.

The RN will document changes and communicate with case managers and families while delivering high-quality, compassionate care. A valid RN license and transportation are required.

Qualifications

• RN license in practicing state is required.
• Graduate of an approved school of professional nursing.
• Minimum 1 year of clinical home health experience preferred.
• Must have a valid driver's license and reliable transportation.
• CPR certification preferred.

Responsibilities

• Follows the Care Plan in collaboration with the Director of Nursing and care team.
• Conducts on-site orientations and supervision visits for home care aides.
• Provides ongoing instructions to home health aides during visits.
• Reviews Personal Care Plans from ASAP and orients aides accordingly.
• Documents services rendered and changes in client conditions.
• Communicates with ASAP Case Managers or family as needed.
• Performs other duties as assigned.
