Job summary

Enlyte is seeking a part-time Field Case Manager (RN) to work in a hybrid, 20-hour week role in the Marquette, MI area. You will coordinate care with physicians, employers, and patients to develop cost-effective treatment plans that support safe return-to-work.

Qualifications

• Education: Associates Degree or Bachelor’s Degree in Nursing or related field.
• Experience: 2+ years clinical practice preferred.
• Skills: Ability to advocate recommendations effectively with physicians/providers, employers, and customers. Ability to work independently. Knowledge of basic computer skills including Excel, Word, and Outlook Email.
• Certifications, Licenses, Registrations: Active RN license in good standing.
• Travel: Must have reliable transportation and be able to travel to and attend in-person appointments with injured workers in assigned geography.
• Internet: Must have reliable internet.
• Transportation: Must have reliable transportation and be able to travel to and attend in-person appointments with injured workers in assigned geography.

Responsibilities

• Demonstrate knowledge, skills, and competency in case management standards of practice.
• Advise on treatment plans using knowledge of injuries, medications, comorbidities, and job duties.
• Interview injured persons to assess recovery and occupational factors.
• Collaborate with physicians/providers to tailor treatment plans.
• Modify job duties with employers to facilitate early return to work.
• Evaluate and modify case goals based on progress.
• Independently manage workload and prioritize cases.
• Attend appointments and prepare status updates for customers.
